Grant Description

Vision

Our vision for the Leeds Innovation Hub is to bring our EPSRC and NIHR research communities together with the statutory NHS body responsible for commissioning Leeds services, to improve the real-world experiences and quality of life for people with Multiple Long-Term Conditions (MLTCs), while also increasing equality and equity in the design and delivery of health and care services. The ultimate goal is to inform and design new effective integrated models of care, personalised to the needs of people living with MLTCs.

Background

A specific Long-Term Conditions Population Board (LTC-PB) has already been established by the NHS West Yorkshire Integrated Care Board, and includes a comprehensive group of health and social care stakeholders. The LTC-PB has identified a cohort of people living with MLTCs in some of the most deprived areas of Leeds that have the worst health outcomes.

The Leeds academic environment includes multiple research leaders across pertinent fields of systems engineering and healthcare, and we have a strong existing PPIE research network. Aims and Objectives

This development phase aims to build a network of multi-disciplinary researchers (with systems engineering, mathematics, information technology, healthcare and health economic experience) to work with the LTC-PB. It will develop the process for embedding PPIE, beyond that already within the LTC-PB. We will pilot a systems approach in the identified MLTC population to understand experiences of current service offerings and identify needs for ongoing and future service interventions.

These requirements will inform a design hackathon where future service scenarios will be identified and low fidelity prototypes mocked up. We will evaluate hackathon outcomes with our stakeholders. The pilot will inform the key Hub design features, including development of effective ways of working between LTC-PB and academics, and the establishment of appropriate governance. It will enable co-creation of a refined Innovation Hub.

Methods

The development phase will comprise three parts in conjunction with the LTC-PB and other stakeholders, and focus on the already identified patient group with 3+LTCs and at least one mental health condition in the most deprived areas of Leeds. Part 1 (Months 1-3) will establish PPIE methodology, governance and organisation structures for the pilot and gain the necessary approvals.

In addition, we will initiate a strategy for the wider context within which the Hub will operate. Part 2 (Months 4-15) will deliver the systems engineering pilot in conjunction with patients and other stakeholders from the target population. Part 3 (Months 16-18) will build on learning from the pilot and will include an All-Hands workshop to review what did and did not work well and co-create a high-level design for the full Hub, including a refined vision, effective ways of working, the PPIE methodology, and other target populations of MLTCs.

Timelines and milestones

By its nature, our collaboration between academics and the statutory NHS body provides a unique opportunity to substantially impact health and social care systems with rapid implementation. Dissemination of the Hub development phase will be iterative and we will hold a dissemination workshop for all stakeholders
