Purpose and goal: Tyresö municipality, the Region and Collabodoc intend to implement a digital

collaboration platform, where different professions can communicate about the individual regardless of the principal in order to shorten lead times, increase patient safety and create traceability in communication. The project is expected to reach the objective that was set with the individual/patient in focus, while the project will place great emphasis on user-friendliness in the platform and efficient processes for all professions.

The project intends to spread the results and benefits of the project with, for example, other municipalities with the same challenges.
