**Project Title:** Empowering Families, Transforming Epilepsy Care: A Digital Solution for Children and Young People in West Yorkshire **Public Description:**
Epilepsy is a common neurological condition affecting thousands of children and young people (CYP) in West Yorkshire. Managing epilepsy can be challenging, especially for those with complex cases requiring specialized care. Families often grapple with navigating a fragmented healthcare system, accessing timely support, and understanding the complexities of their child's condition.
EnrichMyCare, an innovative digital health platform, aims to transform epilepsy care for CYP in West Yorkshire by empowering families, streamlining communication, and improving access to vital information and resources. **Our platform offers:**
A Centralized Hub: Securely storing health and care records, seizure diaries, medication information, and educational resources in one accessible place.
Personalised Care Plans: Tailored to each child's unique needs, empowering families to actively manage their child's condition and track progress over time.
Enhanced Communication: Facilitating seamless communication between families, clinicians, and educators, ensuring coordinated and informed care.
Data-Driven Insights: Utilizing advanced analytics to identify patterns in seizure activity, medication responses, and triggers, enabling more personalized treatment plans and proactive care management.
Community Support: Connecting families with a supportive online community and resources to help them navigate the challenges of epilepsy.
By partnering with Leeds Teaching Hospitals NHS Trust, a leading center for epilepsy care, we will ensure EnrichMyCare is tailored to the specific needs of the West Yorkshire region. Our pilot study will rigorously evaluate the platform's impact on patient outcomes, family well-being, and healthcare resource utilization.
This project promises to make a real difference in the lives of CYP with epilepsy and their families, offering a user-friendly, accessible, and comprehensive solution that empowers them to take control of their health and well-being.
